GCDC Enrollment Forms

Welcome to GCDC. We are happy to have your family as a part of our program. This website provides information about the GCDC enrollment requirements.  If you should have any questions, please do not hesitate to contact us at (301) 286-8588 /  info@gcdc-connect.org .

All documents must be printed and provided to the school
ONE WEEK prior to enrollment.  
(We will not print out documents that have been emailed.)

Step 1: Accept the Enrollment Offer Within Two Business Days

Submit the following within two business days:

A completed acceptance form.  The acceptance form.   Download form here .

A copy of the child’s birth certificate. NASA Goddard Child Development Center (GCDC) is ONLY open to children of Goddard Space Flight Center (GSFC) Civil Servants and Contractors. We have a limited number of spaces open to Civil Servants of other Government Agencies and employees of the University of Maryland. Without proof of legal custody, other family members (e.g., grandchildren, nieces, nephews) GCDC will not enroll the child regardless of time spent on the waiting list.

Proof of eligibility to use our program. A copy of valid identification (e.g. NASA ID, ID from other federal agencies, copy valid University of Maryland faculty identification, etc.)

Tuition Express Form. We will use this to collect your two weeks deposit and collect your child’s tuition throughout enrollment.    Download form here  Please note, families must submit all of the documents listed in step one within two business days of being offered a space in our program. Without it, we will move to the next person on the list.  

NASA Locator and Information Services Tracking System (LISTS) Data Form.  For non-GSFC parents who will need access to GSFC.  Download form here.  Complete highlighted sections only. This information is for the person being badged. Volunteer Requirements  Provide with this form, a copy of your driver’s license or other eligibility documents must be submitted to process the badge request.

Submit Everything in Step 1 within 48 hours.


Step 2: Schedule Visit Days

Please call to schedule transitional visit hours for your child.


Step 3: Make an Appointment With Your Child’s Physician

Make an appointment with your child’s physician to have the forms listed completed. Note: We require these specific forms. Alternate forms will not be accepted. Forms marked this symbol (**) are required for all children. Other forms are needed when medically necessary.  

** Child Health Inventory Form :    All enrolled families are required to provide a physician signed and stamped health assessment for their child annually. Please ensure that the sections for TB Skin Test and Lead Testing are not left blank. A TB skin test is required every other year. GCDC needs the lead form for all students (regardless of zip code).  Leaving these sections blank will result in the form having to be returned to your child’s physician to have those fields addressed.

** Immunization Report:  Your physician may complete this form or provide a computer generated listing of immunizations.

** Lead Form : This is a required form.

The following forms are required if applicable to your child.

Seizure Medication Form:  Children diagnosed with seizures are required to have this form completed by this physician. It is likely that if your child is prone to seizures, your child’s physician will prescribe rescue medication. GCDC requires a medication form for any seizure medications kept at the school; therefore, please ensure that your child’s physician completes a medication form as well.

Medication Form:   GCDC only administers emergency medications (e.g. EpiPens, inhalers, Benadryl, etc.). If your child has a medical condition that requires rescue/ life-saving medication, a medication form is needed for each mediation maintained at GCDC.

Allergy Action Plan:   Children with food allergies must have an Allergy Action Plan on file at GCDC. It is likely that if your child has a food allergy, your child’s physician will prescribe a form of rescue medication. GCDC requires a medication form for each prescription (e.g. Benadryl or EpiPens) kept at school. Please ensure that your child’s physician completes a medication form as well.

Asthma Action Plan:  Children diagnosed with asthma are required to have an Asthma Action Plan on file. It is likely that if your child has asthma, your child’s physician will prescribe a rescue medication. A medication form is required for inhalers kept at school. Please ensure that your child’s physician completes a medication form as well

Step 4: Complete Enrollment Forms

Parents must print and provide hard copies of each form. Links to all required forms are below, including those mentioned in Steps 1-3.   Please take note of special instructions for the Emergency Authorized Medical Treatment & Emergency Contact Forms.

Step 5: Complete Additional Requirements

Please provide a copy of your child’s health insurance card coverage. This is required for treatment in an emergency.

Provide one family photo for the classroom’s family tree.

Please note: Children must have a complete, printed-out registration packet at least ONE WEEK before the first day . Because of the amount of time it takes to process an enrollment packet, we cannot accept packets on the first day of attendance. A complete registration packet includes every form on this page. 

We will not print out documents that have been emailed.

Please submit them all at once to ensure that all documents are retrieved and filed accordingly. ​
